I seek an explanation as to why men have surgery( prostatectomy ) in the presence of high Gleason scores and lymph nodes nodes show on PET scans

I had nothing show on the PET scan, but surgery found it in the lymph nodes. I was under the impression that once there is detected spread outside the prostate that surgery wasn’t generally done.. that is was like closing the barn door after the horse has bolted.
